Job Title : Social Worker

Location : Warwick, RI, United States, 02886

Shift : 8.30am to 4.30pm, Monday to Friday

Schedule : 5 days a week - 40 hours

Roles and Responsibilities :

As a key member of our interdisciplinary care team, you’ll support patients and families by providing emotional guidance, resource coordination, and advocacy in the hospice settings.

You’ll collaborate closely with nurses, therapists, and physicians to ensure comprehensive care and a strong support system for every patient.

Offering emotional and psychosocial support to patients and families

Helping patients and caregivers set healthy boundaries and goals

Identifying high-risk situations and coordinating timely interventions

Referring families to community services and financial assistance programs

Supporting hospice patients with bereavement and respite care planning

Participating in team case conferences and care planning

Providing supervision to other social work staff (if licensed at LICSW level)
