primary care operations team, the Primary Care Nurse works directly
with Mercy Primary Care Providers to provide safe, timely,
effective, efficient, equitable, patient-centered care in a
systematic way. The Primary Care Nurse will partner with patients,
providers, and other members of the clinical care team to drive
quality outcomes for an attributed panel.Position Details : Minimum
Qualifications : Licensure : Graduate of an accredited practical
nursing program; Consideration will be given for comparable
education experience and successful licensure. Is personally
responsible for obtaining and maintaining a current LPN license
within the hiring state in which nursing duties are performed and
must meet all State Board of Nursing requirements. Preferred
Qualifications : Certification(s) : NOTE : one or more of the
certifications below may be required based on the position / unit
hired to, or acquisition of certification within department
