About The Position

Position Summary :

Seeking an experienced Home Health Physical Therapist to join a dedicated team delivering in-home care across the greater West Portland region. The PT will provide skilled therapy services focused on restoring function, mobility, and independence. This role requires strong documentation skills, comfort working independently, and familiarity with Oasis and Epic.

Key Responsibilities :

Perform in-home PT evaluations and develop individualized treatment plans

Deliver interventions including gait training, strengthening, and functional mobility strategies

Document visits and progress using Oasis and EPIC

Collaborate with interdisciplinary care teams to achieve optimal outcomes

Travel to patient homes (up to 60 miles / day); mileage reimbursed at IRS standard rate

Maintain a productivity expectation of up to 6 patients / day or 30 points / day

Job Requirements

Required Qualifications :

Graduate of an accredited Physical Therapy program

Active Oregon PT license (pending accepted)

Minimum 1 year of skilled Home Health experience

Oasis experience required

BLS certification (AHA or Red Cross)

Reliable personal transportation

Preferred Skills :

Experience with Epic EMR

Strong time management and autonomy in the field

Effective communication and patient education abilities
