Role Summary : Position is for a licensed Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to lead our cardiology team in a transformative initiative aimed at improving outcomes for heart failure patients. This role is central to reducing hospital readmissions, enhancing quality of life, and supporting physician workflows through a multidisciplinary, patient-centered care model. Physician Workflows : Integrate and supplement primary care and heart failure cardiology clinical visits in the management of heart failure patients. Key Responsibilities :
- Conduct ambulatory visits for HF patients, serving as the primary cardiology-based NP / PA
- Provide early post-discharge follow-up (within 3–5 days) to ensure continuity of care
- Collaborate with cardiologists, nurses, pharmacists, and care coordinators in a multidisciplinary team.
- Utilize remote monitoring tools and telehealth platforms for high-risk patient surveillance.
- Educate patients and families on medication adherence, dietary guidelines, and symptom recognition.
- Coordinate referrals to ancillary services including cardiac rehab, dietary services, discharge planning, and social work
- Participate in patient education programs and workshops to promote self-care and family engagement.
- Support palliative care efforts including symptom management and advance care planning
- Document all clinical interactions and care plans in the EMR with accuracy and timeliness
- Monitor and report on quality metrics related to readmission rates and patient outcomes

Licensed Nurse Practitioner or Physician Assistant in California with national certificationMinimum of 2–3 years of clinical experience, preferably in cardiology or critical careProficiency in EMR systems and remote monitoring platforms; Epic preferredStrong communication, organizational, and patient engagement skillsBLS / ACLS certification (or willingness to obtain)Benefits :
