Palliative RN Clinical Navigator, M-F, 8-4 : 30
Position Summary : The Palliative RN provides episodic advanced serious disease management, urgent visits, promotes preventative care and wellness, pain management, and augments the provision of end of life / palliative care. Key responsibilities include assessing patient needs, coordinating consults, providing education, liaising with various care providers and community resources, and maintaining accurate documentation.

Compensation based on experience $45.00 - $47.00 per hour
