- Have only one valid CLD & current DOT medical card not expiring in the next 90 days
- No DWI / DUI or criminal offense in the last 7 years
- Have a present CDL for the past 24 months with a minimum of 24 months verified in the last 36 months operating the same type of vehicle to be insured
- Must be at minimum 25 years old
- Must accept that we are in the National Drug & Alcohol Clearinghouse. ( NOTE : NDAC will release positive results in the past 3 years)
- Must have a CSA score of less than 100 for the past 2 years. ( NOTE : drivers with scores from 51-99 are evaluated on an individual basis and may be accepted on probation.)
- Truck must be manufactured in the year 2000 or sooner
- The engine must be modeled for the year 2000 or newer and capable of ELD compatibility Powered by JazzHR
