Job Title : Complex Care Social Worker

Schedule : Full-time, Day shift, 8 : 00am - 4 : 30pm

Job Responsibilities :

• Facilitate discharge planning aligned with goals for length of stay, focusing on difficult-to-place, high-cost, and escalated patients.
• Utilize a multi-step escalation protocol to identify patients with barriers to effective discharge within the targeted length of stay.
• Collaborate with the interdisciplinary team to manage escalated patients according to discharge planning standards.
• Monitor high-cost and difficult-to-place patients to identify early intervention opportunities, enabling timely and cost-effective discharges within utilization review guidelines.
• Assist community patients by referring them to resources and addressing barriers.
