Must-Have : Flu vaccine required, no exemptions.

Description :

Under the general direction of the Director / Assistant Director / Manager of Pharmacy Services, the Medical Social Worker provides indirect patient care.

The Medical Social Worker collaborates with nurses, peers, physicians, and other members of the healthcare team in the delivery of services to patients.

The Medical Social Worker reviews patient information and collaborates on care plans in accordance with policies and procedures.

The Medical Social Worker demonstrates leadership and management skills and a commitment to customer service and professional growth.

The Medical Social Worker completes a comprehensive initial assessment.

The Medical Social Worker performs all activities for multidisciplinary care coordination within and between hospitals.

The Medical Social Worker engages in transitional planning activities that include patient and caregiver education and aftercare arrangements.

The Medical Social Worker communicates and collaborates relevant patient information with appropriate team members.

The Medical Social Worker ensures department goals related to length of stay reduction, readmission prevention, denial reduction, and satisfaction improvement are met.

The Medical Social Worker documents per department requirements.

The Medical Social Worker participates in Performance Improvement activities as needed.

The Medical Social Worker performs all other duties as assigned.
