Nurse / Care Manager Level 2 – Boys Community Residence

I. QUALIFICATIONS
NYS Registered Nurse License, BSN preferred
Two years’ professional nursing experienceOne year experience with DD / MH population preferredSkillsInterpersonal, organizational and communication skills
Ability to instruct staff and individuals in basic health careAbility to work with the Interdisciplinary Treatment TeamFamiliar with community agencies and resourcesFamiliar with computer programs (e.g. Word, Excel, EHR)Ability to legally operate a vehicle in NYSThe ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighting up to 40 lbs.II. III . MAJOR FUNCTIONS
Primary Duties
Work as part of a Care Coordination team
Assist / cover for other nursing staff absencesProvide medication education to staff and individualsParticipate in multidisciplinary team meetings and case reviewsIdentify and advocate for individual rightsTeach / assist in developing / maintaining ADL skillsTeach medication administration / supervision to staffCertify DD staff to pass medicationsConduct initial and ongoing assessmentsMonitor routine health care of all individualsProvide / arrange for routine and emergency healthcareReview all health-related incidentsOversee / order all medicationsMaintain health records according to standards and regulationsProvide in-service training regarding health issuesMaintain / coordinate effective and on-going communication with other service providersParticipate in the development / documentation / review and update of care planAct as linkage to community servicesCoordinate and conduct care planning activities for a designated case load of recipientsMay utilize health information technology to coordinate care and communicate with providersAct as medical resource consultant for team membersPromote health and wellness activities for recipients via individual or group sessionsProvide crisis intervention services as necessaryProvide transportation as needed to necessary servicesPromote self-advocacy and personal empowermentMay be assigned to participate in rotating afterhours / weekend on call coverageEstablish relationship with all individuals, family members, and advocatesProvide blood borne pathogens and other training to program and agency staff as neededUnderstand and follow all program policies and proceduresRepresent the agency and program in the community and with other service providersParticipate in and successfully complete all required staff training and development activitiesRegular and substantial contact with individuals receiving servicesPerform other related tasks as directed by supervisorIn compliance with all applicable government laws, rules, regulations and standards and all Catholic Charities Policies and Procedures the duties and responsibilities of this position include but are not limited to the above.
