I.         QUALIFICATIONS

• Education / Experience

NYS Registered Nurse License, BSN preferred

Interpersonal, organizational and communication skills

II.        RESPONSIBLE TO :             Supervising Nurse / Care Manager

III .      MAJOR FUNCTIONS

Primary Duties

Work as part of a Care Coordination team
