GENERAL DESCRIPTION OF THE POSITION :

The Case Manager / Driver at Tatum Psychology Employment Group is responsible for overseeing documentation of patient’s intake, progress, and discharge, as well as liking them with appropriate social, occupational, and educational resources as needed. The Case Manager works well with the treatment team to plan and coordinate the client’s progress through the continuum of care.

ESSENTIAL FUNCTIONS :

1. Patient Admission & Orientation

• Admits new patients by completing the intake process and new patient orientation.

2. Assessment & Treatment Planning

3. Case Management & Patient Monitoring

4. Communication & Interdisciplinary Collaboration

5. Discharge Planning & Post-Care Coordination

6. Staff Development & Training

7. Transportation
