About this Job : We are seeking a passionate Bilingual Case Manager to join our Case Management Team, dedicated to providing high-quality, patient-centered care for our Breast and Cervical Cancer Outpatient Program. This role is essential in guiding patients through their care journey from pre-admission to post-discharge, ensuring tailored approaches to meet individual needs.

Key Responsibilities :

• Contribute to department goals and adhere to policies, procedures, and safety standards.
• Work collaboratively with a multidisciplinary healthcare team to develop and implement comprehensive care plans.
• Maintain open communication with healthcare providers and families about care progress, challenges, and discharge planning.
• Conduct thorough assessments to identify patients in need of case management based on high-risk indicators.
• Demonstrate expertise in addressing the biopsychosocial needs of patients.
• Educate patients and families, equipping them with the knowledge needed for effective recovery and care transitions.
• Evaluate patient outcomes and modify care plans as necessary to enhance effectiveness.
• Manage a caseload of patients, focusing on their care from admission to post-discharge follow-up.
• Stay updated on managed care trends, healthcare financial practices, and relevant regulations.
• Participate actively in performance improvement initiatives and program evaluations.
• Serve as a resource for other case managers and healthcare staff, providing guidance and support.
• Ensure timely documentation of all case management activities and adherence to confidentiality regulations.
• Engage in community outreach efforts and represent the department in various capacities.
