Mental Health Clinician - Consortium Grant

Position Objective :

The Licensed Clinician will work in tandem with the primary care team and is responsible for providing brief intervention and consultation to patients in an outpatient setting. Services to include : crisis intervention; monitoring mental health referrals to patients with higher behavioral health needs; coordinating care for complex patients with mental and physical health conditions; acting as a consultant and resource to members of the care team on effective interventions with patients with chronic illnesses and mental health diagnoses; assisting with staff training as needed; functioning as chronic care self-management support expert; meeting minimum documentation requirements for all services rendered in a timely manner; and ensuring data is collected as appropriate.

Patient Care Management / Documentation

• Work collaboratively with physician and / or mental health provider of record to provide comprehensive mental health services to patients across the age spectrum.
• Develop and implement an integrated and comprehensive treatment plan based on clinical experience and knowledge.
• Maintain a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
• Maintain confidentiality and patient privacy.
• Document care provided; meeting standards for chart reviews.
• Adhere to all practice standards, protocols and regulations set by COMAR / Joint Commission.

Physical Demands -

Light work - Exerting up to 20 pounds of force occasionally, and / or up to 10 pounds of force frequently, and / or a negligible amount of force constantly to move objects. If the use of arm and / or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.

Pay Range

$19 - $23 USD
