JOB COACH ( Nassau/Suffolk County)

Part Time positions available.

• The pay rate for this position is $21.50 per hour.
• The shift for this position is Tuesday-Saturday or Sunday-Thursday

You must have have a valid driver's license and a HS diploma/GED to be considered for this position. Some experience with the IDD population is preferred but not required.

Qualifications & Responsibilities

• Some experience with the IDD population is preferred but not required.
• Responsible for fulfilling Life’s WORC core values, and ensuring for person-centered approach to Customized Employment.
• Teach soft skills and appropriate workplace behaviors to individuals supported.
• Provide transportation to individuals participating in the community based prevocational program.
• Assist individuals acclimate to the workforce, and identify strategies to help each individual prepare for future employment.
• Serve as liaison between business partners, individual, family and agency, while maintaining professional, positive relationship with partners.
• Complete daily case notes and monthly summary notes
• Ensure regulatory compliance and agency policy and procedures with regard to incident reporting.
• Perform other duties, as assigned.
