Job description

Artemis Healthcare Partners is seeking a dedicated and passionate professional to join our client's team!

Position Name : Registered Nurse (RN) - Home Health and Hospice

Location : Evergreen, CO

Employment Type : Direct-Hire & Permanent

Setting : Home Health and Hospice

Shift : Full-Time

Compensation :

  • Pay : $40.00-$48.00 per hour
  • Sign-On Bonus : $5,000
  • On-call compensation structure (availability pay plus hourly rate for visits)

Qualifications : Required :

  • Graduate of an accredited school of nursing.
  • Current RN licensure in Colorado.
  • CPR certification upon hire.
  • Valid driver's license with reliable, insured vehicle for community travel.
  • Strong assessment, communication, and clinical documentation skills.
  • Ability to work independently and manage patient caseload effectively.
  • Position serves both home health and hospice patients with flexibility between service lines.
  • Travel throughout mountain communities required, especially challenging in winter conditions.
  • On-call rotation shared among nursing team.
  • Preferred :

  • Bachelor's Degree in Nursing (BSN).
  • 1-2 years of recent acute care experience in institutional setting.
  • 1-2 years of recent home health or hospice experience.
  • Experience with both home health and hospice service lines.
  • Familiarity with rural healthcare delivery and mountain community needs.
  • Job Summary :

  • Provide comprehensive nursing care to patients and families in their homes across home health and hospice service lines.
  • Conduct initial and ongoing assessments to determine patient and family care needs.
  • Develop, implement, and regularly evaluate individualized care plans based on patient status and goals.
  • Deliver skilled nursing interventions including medication administration, wound care, and treatments per physician orders.
  • Serve as case manager for assigned caseload, coordinating care with interdisciplinary team members.
  • Educate patients and families on self-care techniques, medication management, and health maintenance.
  • Collaborate with physicians, therapists, and community resources to optimize patient outcomes.
  • Maintain accurate clinical documentation and communicate changes in patient condition to care team.
  • Support patients and families emotionally throughout care continuum, particularly in end-of-life care.
  • Participate in on-call rotation providing after-hours patient support (reimbursed at Colorado minimum wage for availability, hourly rate for patient visits).
