New York Licensed Behavioral Health RN

A company is looking for a Behavioral Health RN Case Manager - Remote.

Key Responsibilities

Make outbound and receive inbound calls to assess members' current health status

Identify gaps or barriers in treatment plans and provide patient education for self-management

Coordinate care and make referrals to outside resources while ensuring a complete continuum of quality care

Required Qualifications

Current, unrestricted Compact RN license in the state of residence

New York single state license or willingness to obtain within 90 days of employment

1+ years of experience with mental health / behavioral health / substance use focus

Proficiency in Microsoft Office Tools and Systems
