Shift : Monday - Friday 8 am to 5 : 00 pm

Status : Full time

Location : LIFE Kinder Park, 1857 Constitution Ave, Woodlyn PA.

Position Purpose :

Under the supervision of the medical director, the Primary Care physician (PCP) provides primary medical care to PACE Organization (PO) participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan of care. Functions in a collegial relationship with the other health care professionals, making independent decisions regarding medical needs and health regimens. Carries out medical procedures as indicated.

Position Details :

This position will be housed at the LIFE Kinder Park.

What you will do :

Maintain department budget, place orders for equipment and supplies as necessary. Identify financial vulnerabilities and make cost reduction recommendations as needed.

Evaluate, develop, mentor, coach, counsel and discipline department staff. Supports other personnel from other departments through coaching and mentoring to help achieve optimum standard of excellence. Addresses issues of concern through courageous conversation and notifies department manager of any interactions requiring attention.

Maintains a working knowledge of applicable Federal, State, and local laws and regulations, TH PACE Corporate Integrity Program, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.

In light of a disaster will know your disaster recovery, crisis management and business continuity plans and act within your role that is developed within the business continuity plan. Which may include working at another location, remotely from home, and maintaining constant contact with key personnel.

Attend and participate in scheduled training, in-service training, mandatory annual in-service training and educational classes as required / needed.

Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, and unscheduled,; attend morning updates and report changes in participants' baseline status to appropriate staff on a daily basis.

Provide initial and ongoing participant assessments, care plan development, and implementation of care plan.

Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team. Interacts with team members to meet emergent and acute need of participants. Evaluates and treats participants Primary Care Physician during acute illness.

Manages all care of participants in the nursing home : Provides regular visits to manage the care of participants in the nursing home in collaboration with the Interdisciplinary Team; maintains communication with nursing home staff for continuous monitoring of PACE participants on a case-by-case basis; admits nursing home participants to hospital when necessary. Oversees and manages participant's use of medical specialist and inpatient care. Communicates and follows up with specialist regarding outcome of visit.

Function as a member of the interdisciplinary team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24-hour care delivery.

At the direction of the Medical Director reviews medical outcomes / statistical and utilization data, including reviewing of necessity of hospitalizations, and use of specialists. When problems are identified he / she works with the Medical Director to establish best practices and works with clinical staff and interdisciplinary team to make appropriate procedural and or operational changes to avoid unnecessary hospitalizations and improve the health of participants.

Complete documentation timely and uses appropriate diagnostic coding. Respond to coding department in a timely manner.

Advise the Medical Director in ways and means to establish better accountability of PO services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.

Assist with the development of policies and procedure, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.

Participate in provider on-call rotation as required.

Minimum Qualifications :

Doctorate Degree (Includes PhDs) : MD or DO. Current and active plenary physician license to practice in state of PO required. CDS Certification and DEA Registration required. Current BLS required.

Board Certification Internal Medicine or Family Practice required. Board Certification in Geriatrics preferred.

A minimum of one (1) year working with the frail or elderly population required.

Must have a valid driver's license, proof of insurance and have means of transportation.

Must be action-oriented, have business acumen, manage conflict well, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility. Ability to work with minimal supervision and exercise independent judgment.
