Delaware Licensed Pharmacist

A company is looking for a Pharmacist, Medication Management, Remote.

Key Responsibilities

Leverage data to identify patients not meeting quality measures and collaborate with practices to overcome barriers

Perform comprehensive medication reviews during transitions of care and make patient-centered medication recommendations

Develop and deliver learning materials for clinicians, practices, and patients to address knowledge gaps related to quality measures

Required Qualifications

Doctor of Pharmacy (PharmD) degree or Bachelor's degree in Pharmacy

Must be licensed in the state of practice

Graduate of an accredited School of Pharmacy

Completion of a pharmacy residency or equivalent (3 or more years of experience in value-based care or community pharmacy practice)

In-depth knowledge of medication adherence, medication spend, and pharmacy quality measures
