Reports to : Clinic Nurse Manager

Job Summary / Objective :

The Clinic Float Nurse (RN or LPN) provides flexible clinical support across multiple HCCH Medical Clinic locations, including Bethany, Eagleville, and Pattonsburg. This role fills in as needed in the roles of Triage Nurse, Clinic Nurse, Medical Assistant (MA), and Clinical Liaison Nurse. The Float Nurse ensures continuity of care, consistent communication, and quality service regardless of assignment.

Essential Functions / Duties / Responsibilities :

Triage Nurse Duties :

Assess and prioritize patient symptoms and concerns via phone and in-person visits.

Provide appropriate medical advice based on established protocols.

Coordinate care by scheduling timely appointments or directing patients to urgent / emergency care as needed.

Document triage interactions accurately in the EMR.

Clinic Nurse Duties :

Administer medications, immunizations, and treatments as ordered by the provider.

Assist with in-office procedures, including patient preparation and follow-up care.

Perform diagnostic tests (e.g., EKGs, spirometry) as needed.

Monitor and manage patient flow in collaboration with providers and clinical staff.

Call patients with test results, follow provider instructions, and ensure understanding of next steps or follow-up.

Medical Assistant Duties (as needed) :

Room patients and collect vital signs, medical history, and chief complaints.

Prepare and maintain exam rooms for readiness and cleanliness.

Assist with basic front-office tasks such as check-in / check-out during high-volume periods.

Clinical Liaison Nurse Duties :

Act as a communication bridge between the clinic and hospitals, nursing homes, hospice, and home health agencies.

Coordinate care transitions and share necessary clinical information with external care partners.

Ensure accurate and timely documentation of communication and care coordination.

Skills / Abilities :

• Strong clinical judgment, communication, and interpersonal skills.

Organized, flexible, and able to adapt to different clinic settings and roles.

Proficient in EMR usage and basic computer skills.

Education, Certificates, and License :

Education :

Graduate of an accredited nursing program.

Certificates / License :

Current and unencumbered RN or LPN license in the State of Missouri.

Certifications :

Basic Life Support (BLS), active or within 30 days of hire.

Experience :

Previous outpatient clinic experience preferred.

Experience with triage, care coordination, and patient communication is beneficial.

Physical Requirements :

Mobility : Travel between HCCH Medical Clinics in Bethany, Eagleville, and Pattonsburg is required.

Posture : Frequent standing, walking, and multitasking in a fast-paced environment.
