Aging Case Aide 2 (Local Government) - Lebanon County Area Agency on Aging

THE POSITION

Working closely with older adults and their families and assisting them in navigating a variety of support systems is key for enhancing their quality of life. DESCRIPTION OF WORK

In this position, you will participate in crucial work assessing the needs of our clients, connecting them with appropriate resources, and ensuring they receive the assistance required to maintain their independence and dignity. Your main tasks will involve managing information and referral calls, directing inquiries to the right staff, and organizing physical files. You will also provide support to OPTIONS care managers during their monthly telephone contacts at the 3- and 9-month intervals, assist the PA Medi Coordinator with program-related activities, and manage the 6-month and annual assessments for Domiciliary Care plans, along with any additional duties assigned. Work Schedule and Additional Information :

• Full-time employment
• Work hours are 8 : 00 AM to 4 : 30 PM, Monday - Friday, with a 60-minute lunch.
• Telework : You will not have the option to telework in this position.
• Salary : In some cases, the starting salary may be non-negotiable.
