Job description

Description

The Post-Acute Coordinator is responsible for overseeing, executing, and verifying post-acute transition plans for patients. This role will address requests for post-acute services from both inpatient clinicians and community-based providers.

    • What You Need

  • High school diploma or equivalent is required.
  • 2 yrs of an equivalent combination of education and experience in a hospital / medical related field is required.
