Licensed Nurse LPN or RN

Hanover Terrace Health & Rehabilitation Center is looking for compassionate and motivated RN or LPN to join our Team!!

We are looking for a nurse who want to join a team committed to delivering the highest quality of care possible while facilitating a home like environment to the residents / patients in our care.  Our facility offers a variety of services to our customers.   Short-term rehabilitation on a post acute unit, long term custodial care, as well as memory care services.

General Duties :

• Prepare and Administer medications ordered by MD
• Assess / Evaluate resident / patient and document / report accordingly
• Assist in developing and implementing comprehensive plan of care to facilitate the highest level of functional and emotional well-being for the customer
• Perform wound care as ordered
• Communicate and Collaborate with patients and family members
• Thorough and comprehensive documentation in electronic medical record
