Position : Primary Care Physician
Pay : & $260,000.00 - $312,000.00 per year
Location : Jacksonville, Florida, Pennsylvania, North Carolina, Arizona, Ohio, Wisconsin, Missouri, Arkansas
Position & Type : & Full-Time
Shift : & Monday-Friday 8 : 00 AM – 5 : 00 PM
Compensation :
- Base Salary : $260,000
- Bonus : Up to 20% of base salary based on :
- Patient satisfaction ratings
- Alignment with organizational values
- HEDIS compliance metrics
- Reduction of hospital sick days
- Model : Value-Based Care (Outpatient)
- Patient Population : Seniors with 3–5 chronic conditions (e.g., heart disease, diabetes, hypertension)
- Patient Load : 10–12 patients per day
- Panel Size : Maximum of 650
- Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM
- Call Schedule : Based on market provider count (e.g., 1 : 32 weeks if 32 physicians in region)
- Care Team Support : Each physician is partnered with a Medical Assistant, Scheduler, Referral Specialist, and full administrative and leadership support
