Nurse Practitioner / Physician Assistant (Post-Acute Care)
Full-Time
Salary $120,000 – $140,000 per year
About the Role
We are seeking a compassionate and skilled Nurse Practitioner (FNP / AGNP) or Physician Assistant (PA) to join our dedicated post-acute care team. You’ll play a key role in improving outcomes for patients transitioning from hospital to skilled nursing or long-term acute care settings.
Responsibilities
- Conduct comprehensive patient assessments and review medical histories
- Order, interpret, and follow up on diagnostic tests (labs, imaging, etc.)
- Diagnose and manage acute, chronic, and complex medical conditions
- Develop and implement individualized treatment plans, including medication management
- Collaborate closely with attending physicians, rehab specialists, and nursing staff
- Educate patients and families on diagnoses, medications, and preventive care
- Maintain accurate and timely documentation in our EMR (RXNT)
- Advocate for health promotion and disease prevention within post-acute environments
What We’re Looking For
Proven experience as a Nurse Practitioner or Physician Assistant in post-acute, hospitalist, or rehab settingsSkilled in managing multi-morbid and complex patient populations in SNFs or LTACsKnowledge of healthcare regulations and evidence-based patient care standardsExcellent communicator and team collaborator in an interdisciplinary care modelPatient-centered professional with adaptability and a commitment to lifelong learningActive and unrestricted FNP, AGNP, or PA license (Oklahoma preferred or compact eligible)Why Join Us?

Diagnose and treat acute, episodic, or chronic illness, independently or as part of a healthcare team. May focus on health promotion and disease prevention. May order, perform, or interpret diagnostic tests such as lab work and x rays. May prescribe medication. Must be registered nurses who have specialized graduate education.Maintain complete and detailed records of patients' health care plans and prognoses.Develop treatment plans, based on scientific rationale, standards of care, and professional practice guidelines.Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability.Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.Diagnose or treat complex, unstable, comorbid, episodic, or emergency conditions in collaboration with other health care providers as necessary.
