Description
  • Assess patients in inpatient and outpatient environments, creating tailored treatment plans
  • Implement therapeutic interventions focused on enhancing mobility, strength, balance, and overall functionality
  • Work collaboratively with nurses, physicians, case managers, and rehabilitation experts to ensure optimal recovery outcomes
  • Educate patients and their families about treatment objectives, discharge plans, and at-home exercises
  • Keep thorough records of evaluations, treatments, and patient progress in compliance with standards and regulations
  • Engage in team discussions and interdisciplinary meetings to promote coordinated patient care
Education
  • Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution
Certifications
  • Valid state licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional certifications (e.g., in geriatrics or neurology) are advantageous but not mandatory
Skills
  • Preferred experience in a similar role
  • Aptitude for addressing complex medical conditions and mobility challenges
  • Excellent communication and teamwork abilities
  • Proficient with digital charting and electronic medical records systems
