Aspire Health is seeking a Patient Engagement Specialist to support outreach education and ongoing communication with patients. This role focuses on improving patient participation in care programs appointment adherence and overall patient experience through consistent and compassionate engagement.
Conduct outbound and inbound calls to patients to support care programs and services
Assist patients with appointment scheduling reminders and follow-up communication
Provide general education regarding available health services and benefits
Document patient interactions accurately in electronic health record and CRM systems
Identify patient needs and escalate clinical concerns to appropriate care team members
Support care coordination and population health initiatives
Maintain compliance with HIPAA and organizational standards
High school diploma or equivalent; associates or bachelors degree preferred
Experience in healthcare customer service or call center environments preferred
Strong communication and interpersonal skills
Ability to work independently in a remote environment
Basic computer proficiency and ability to learn healthcare systems
Experience working with Medicare or managed care populations
Bilingual (Spanish/English)
Familiarity with EHR or patient engagement platforms
