We are seeking a certified and experienced Primary Care Practitioner, to join our team. The ideal candidate will be a team player, provide compassionate care to patients across all ages and be adaptable in the day-to-day responsibilities. We see an array of ages from infants to Medicare patients, and it is important for the position that the practitioner is comfortable managing a broad spectrum of acute and chronic conditions, preventive care, and wellness education.

Key Responsibilities

• Deliver high-quality, evidence-based primary care to patients of all ages
• Conduct routine physical exams, preventive screenings, and chronic disease management
• Diagnose and treat acute and chronic illnesses
• Order and interpret diagnostic tests and lab results
• Develop and implement individualized treatment plans
• Educate patients on wellness, lifestyle modifications, and disease prevention
• Collaborate with specialists, nurses, and care coordinators to ensure continuity of care
