Role Description

This is a full-time hybrid role for a Quality Outreach Specialist, located in Buffalo, NY. The role is essential for enhancing patient health outcomes by proactively engaging patients to schedule recommended preventive care. The Patient Outreach Specialist will be assigned to multiple primary care practices and will collaborate closely with clinical and leadership staff. The position requires travel to and work from assigned primary care practices in Erie and Niagara County. It will also allow for some work from home or in the office.

Qualifications

• Experience with outreach and communication with patients.
• Basic understanding of HEDIS measures.
• Basic understanding of primary care EMRs.
• Strong analytical skills for tracking and reporting on performance metrics.
• Ability to build relationships with physicians and other members of the care team.
• Ability to work both independently and in a team-oriented, collaborative environment.
• Excellent written, verbal and listening capabilities.
• Associate or Bachelor degree
• Experience working in a medical office environment or patient-facing role.
