Job Title : HYBRID RN - Infectious Disease (Lynn, MA)

Overview :

The RN Case Manager for Infectious Disease supports the provision of coordinated medical and integrated behavioral health services to patients within the Harm Reduction Program. They will be holding a case load of 60-70 patients. For the TB clinic they would be working in a state funded TB clinic (first in the U.S.) where they will be managing patients and helping to do treatments (vaccines / plans). This is a very independent job where the RN will make sure that patients get all proper scans and tests prior to visits. They RN will be doing a lot of the follow up appointments via phone and telehealth. They will be working hand in hand with IV labs, patients and the state.

Responsibilities / Job Duties :

This is an Infectious disease RN Case Manager position with focus primarily on Latent TB and TB disease.

This nurse will be responsible for triaging TB referrals, ensuring that appropriate data is collected and testing is performed before initial visits with providers.

The nurse will do independent follow-up visits, both telemed and in person. There is quite a bit of collaboration with Local Board of Health Nurses , DPH TB division, State TB Lab as well as outside TB Clinic staff.

The nurse will be responsible to assist with sputum collection, appointments, lab testing, and most importantly to keep the patients on track for completion which requires frequent phone calls etc.

The TB Nurse will also be the primary contact person within clinic for questions and education regarding TB.

As an ID nurse, they may also be asked to assist Providers, MA's, CHW's, Case Managers, and our STI councilors and testers with situations that arise in the course of the day, such as vaccines and injections.

Minimum Education and Experience Requirements :

Massachusetts RN License

3+ years of experience

Outpatient Experience

Vaccine Experience

Schedule / Shift :

Monday-Friday - 40 hours / week

Shift : 8 : 30am-5 : 00pm

Required to be onsite for the first 90 days and then will have potential to work 1-2 days / week from home in a hybrid schedule competing paperwork and other duties.
