LanceSoft is seeking a LPN / LVN Home Health for a travel job in Lacey, Washington.

Job Description & Requirements

• 40 hours per week
• Shift : 8 hours, days

Requirements :

Licensure & Certification

Must have a Washington State LVN / LPN license or compact license in-hand at submission.

Experience

Minimum 1 year of recent skilled home health experience (within the last 3 years).

Documentation / System Knowledge

HomeCare HomeBase (HCHB) and Oasis experience required.

Transportation / Travel

Must have reliable personal transportation and a valid driver’s license .

Completion of the Hospice / Home Health Checklist is mandatory.
