Job Description Summary :

MaineGeneral Health is seeking a compassionate and flexible Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team on a per diem basis. This is an excellent opportunity to work within a multidisciplinary team that provides person-centered community-based care to adults living with serious and persistent mental illness.

The ACT Team operates 24 / 7 offering intensive integrated support to help individuals live safely and independently in their communities. As an ACT RN youll play a key role in medication management crisis intervention and ongoing care coordination often meeting clients where they are : in their homes or in the community.

Job Description :

Position : Registered Nurse (RN) - ACT Team

Location : Augusta ME

Schedule : Per diem / Flexible Hours

What Youll Do :

• Plans implements and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors records and communicates patient condition as appropriate.
• Collaborates as needed across disciplines to coordinate patient care including patient transfer admissions discharge referral and spiritual / psychosocial support needs.
• Evaluates the learning needs of the patient and / or family and provides patient / family education appropriate to age culture condition and circumstances.
• Effectively delegates and communicates patient care needs to ancillary personnel. May precept student nurses and new hires.
• Some employees in this job may operate a motor vehicle to conduct MaineGeneral Health business.
