Mantey Heights Rehabilitation & Care Center is seeking a PRN PT to join our amazing therapy team!

About the Role
As our PRN Physical Therapist , you’ll perform initial evaluations for new residents and complete monthly charting and reassessments to ensure each care plan meets residents’ evolving needs.
You’ll work closely with an interdisciplinary team of nursing, occupational, and speech therapy professionals who share your passion for exceptional care.
What You’ll Do
Conduct comprehensive physical therapy evaluations for new admissions.
Develop individualized treatment plans tailored to each resident’s needs.
Complete monthly reassessments, progress notes, and required documentation.
Collaborate with other therapy staff to ensure continuity and quality of care.
Provide education and support to residents, families, and caregivers.
What We’re Looking For
Licensed Physical Therapist (PT) in the State of Colorado.
Skilled nursing or rehab experience preferred.
Strong communication, time management, and documentation skills.
Positive attitude and commitment to resident-centered care.
Reliability and professionalism in a PRN (as-needed) capacity.
Compensation & Perks
Competitive pay : $40.00 – $55.00 per hour DOE
