Flexible Schedule Options Include : 40hrs / 5 days per week; 32 hrs / 4 days per week or Weekend schedule

Key Responsibilities :

  • Collaborate with healthcare professionals during weekly team meetings.
  • Provide rehab and manage medical needs in patients' homes.
  • Administer medications and monitor their effects.
  • Review and adjust care plans and medications as needed.
  • Evaluate and support staff competency in direct care.
  • Communicate patient updates to physicians and the team.
  • Maintain patient records and report incidents as required.
  • Promote patient independence and consumer choice in care.

    • The ideal candidate will have at least 1 year of experience in Home Health, demonstrating a solid understanding of providing patient care in a home setting. We are looking for someone who is compassionate, skilled, and ready to make a meaningful impact on patients' lives.

    • Responsibilities

  • Attends and actively participates in weekly team meetings
  • Provides rehabilitation and coordinates medical needs for clients
  • Medication Monitoring and Administration by oral medication or needle injections
  • Orders, reviews, monitors, and discontinues medications as ordered by the physician
  • Assists with keeping within assigned nursing budget
  • Conducts assessments and approves plan of care and service delivery methods
  • Performs competency and skills evaluations on employees who provide direct care
  • Informs the physician and staff of changes in the client's medical or mental condition and needs
  • Writes progress notes, summaries, and discharges
  • Implements the treatment / rehabilitative activities and strategies
  • Reports significant / major incidents or accidents and notifies proper authorities of circumstances or situations that present possible health hazards
  • Follows consumer health plans and provides and promotes consumer choice and facilitate independence
  • Assists in maintaining and updating nursing, pharmacy, infection control, and health and safety policies and procedures
  • Other tasks as assigned

    • Qualifications

  • Graduate of an accredited school of Nursing
  • Current Registered Nurse licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
  • Bachelor's degree preferred, with one (1) year of home health experience preferred
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist

    USD $35.00 - $50.00 / Hour
