MDS Coordinator - RNAC

The MDS Coordinator plays a critical role in ensuring the quality of patient care through effective medical management and documentation. This position involves overseeing the assessment process for patients in a healthcare setting, ensuring compliance with regulations, and facilitating communication between healthcare providers and patients. • Duties
• Conduct comprehensive assessments of patients to gather necessary information for the Minimum Data Set (MDS) documentation.
• Ensure accurate and timely completion of MDS assessments in compliance with federal and state regulations.
• Collaborate with interdisciplinary teams to develop individualized care plans based on assessment findings.
• Review medical documentation to ensure accuracy and adherence to HIPAA standards.
• Maintain up-to-date knowledge of ICD-9 coding and its application in patient documentation.
• Facilitate communication among healthcare providers, patients, and families regarding care plans and patient progress.
• Participate in quality assurance activities to improve patient outcomes and enhance care delivery processes.
• Provide training and support to staff on MDS processes and documentation requirements.
