Hospice Community Liaison

Blize Healthcare is looking for a Hospice Community Liaison to join our team.

Responsibilities :

• Developing and maintaining relationships with healthcare providers, community organizations, and other referral sources.
• Promoting hospice services through sales calls, trade shows, and other activities.
• Informing the community about hospice / home health care, services, and benefits.
• Tracking and reporting referral and admission data to identify trends and growth opportunities.
• Working with the clinical team to ensure a smooth transition to hospice / home health care.

Required Skills & Qualifications :

Bachelor's degree preferred.

Minimum one year of sales / marketing experience in healthcare.

Ability to market collaboratively and productively with customers, referral sources, and the community.

Excellent written and verbal skills, computer literate.

Strong organizational, people, and problem-solving skills.
