Geriatrics Nurse Practitioner

Job Description:

Our client is seeking a dedicated Nurse Practitioner to provide exceptional patient care in our inpatient rehabilitation facility in Mishawaka, Indiana. The ideal candidate will work collaboratively with our interdisciplinary team to deliver personalized rehabilitation plans and support patient recovery.

Shift: Monday to Friday (8 am to 5 pm) - No on-call/ No weekend

Specialty: Rehabilitation

Setting: Rehab Centre (Inpatient)

Responsibilities:

• Conduct comprehensive evaluations of patients upon admission, including medical history, physical exams, and diagnostic tests.
• Collaborate with the interdisciplinary team to develop and implement individualized rehabilitation plans tailored to each patient's needs.
• Educate patients and their families about rehabilitation processes, treatment plans, and lifestyle modifications to enhance recovery.

Requirements:

• Master's of Science in Nursing - required.
• Active License of Indiana as Nurse Practitioner - required.
• Active BLS/CPR - required
• A year of experience working in Rehab - Preferred.
