Patient Care Coordinator

A company is looking for a Patient Care Coordinator to assist in the daily operations of Clinical Services. Key Responsibilities Act as a liaison between members, providers, and pharmacies or care facilities Support patients in accessing appropriate care, including specialty pharmacy enrollment Coordinate scheduling of specialty medications and provide financial resources to patients Required Qualifications Experience in customer service and teamwork Ability to work independently and meet deadlines Basic understanding of de-escalation techniques Flexibility to adapt to changing priorities Ability to work cooperatively with pharmacy services staff
