$31.40 - $37.44 hourly plus mileage or company vehicle program

Key Responsibilities :

Provide home-based physical therapy treatments under PT guidance

Participate in interdisciplinary patient care efforts

Ensure complete and timely documentation of services

Educate patients and family about therapy processes

Must Have + Certifications :

Minnesota PTA licensure current and valid

Preferred 2 years’ experience

Valid driver’s license and insured vehicle

CPR and BLS certified
