Job Title : Community Transition Coordinator
Status : Full-Time, Non-Exempt
Reports To : Lead Housing and Transition Coordinator
Salary : $20-24 per hour
Position Overview : The Community Transition Coordinator provides information, supports consumers in decision-making, and coordinates multiple services from a variety of organizations to successfully transition older adults and individuals with disabilities from skilled nursing or long-term care facilities to community-based living options.
Essential Functions and Responsibilities :
- Maintains a caseload of a minimum of 14, up to a maximum of 20 active consumers, ensuring consistent engagement and high-quality support tailored to individual needs;
- Conducts outreach and informational presentations at skilled nursing or long-term care facilities to identify and assist individuals who qualify for the program and wish to transition back into the community;
- Using a person-centered approach, interviews consumers to determine eligibility and establish immediate and long-term needs for services;
- Collaborates with the healthcare team, service providers, and other support networks to develop a consumer-driven transition plan taking into account long-term needs;
- Works closely with the Orange County Housing Authority to verify eligibility for the Non-Elderly Disabled (NED) voucher;
- Assists consumers in applying for NED voucher and provides ongoing support throughout the application process, including facilitating extensions as needed;
- Works with community resources and agencies to facilitate a seamless transition, connecting the individual with necessary long-term services and supports;
- Supports consumers in locating and securing suitable housing;
- Provides continued post-placement support for a designated period to promote long-term housing success;
- When needed, assists consumers in applying for funding sources to aid transition including CalAIM, Department of Rehabilitation (DOR) community living fund, etc.;
- Stays informed about available housing vouchers, updates, policy changes, and emerging resources to provide up-to-date information to consumers;
- Enters consumer demographics, case notes, and service activities into a database as part of the monitoring and evaluation process;
- If appropriate, offers peer advice and support to consumers;
- Cross-trains and provides support to consumers of the Personal Assistant Services Program on an as-needed basis;
- Contacts all new transition referrals within 5 business days and provides necessary resources to all consumers who do not qualify for the CalAIM Program;
- Adheres to established office hours and is present to support daily operations while demonstrating flexibility as needed;
- Is adaptable to changing priorities and takes on additional responsibilities as needed.
Minimum Qualifications :
A Bachelor's degree from an accredited college or university;Two years' experience working with individuals with disabilities, in health and human services, or as a case coordinator; or an equivalent combination of education and experience;Experience in sales, particularly in a medical or hospital setting, with a strong ability to establish connections;Demonstrated ability to effectively advocate for services in the health insurance and skilled nursing sectors; or,Demonstrated knowledge of essential services and benefits to facilitate community transition;CalAIM experience preferred but not required;Exhibits strong problem-solving and adaptability during changing situations;Multitasking, organization, and team leadership skills;Evidence of computer literacy;Working knowledge of the Independent Living philosophy and methodology;Capacity to work independently;Compliance with DMC attendance policies;Ability to establish and maintain trusting and effective working relationships with a variety of people from diverse backgrounds;Valid California Driver's License or state identification card, car insurance (if applicable), and reliable mode of transportation;Bilingual and / or capability to communicate in ASL is preferred.Personal Attributes :

While performing the duties of this job, the employee is regularly required to remain in a stationary position working at a desk; operate a computer and other office machinery including printer, copier, and telephone; move about inside the office to access office machinery; exchange information with coworkers and the public; and may need to lift or move up to 10 pounds.
