The home health risk assessment will be conducted on Medicare Advantage plan members residing in Franklin Township, OH,and the surrounding Portage County, OH.Portage County, OH,includes locations like Kent, Ravenna, and StreetsboroCandidates could be scheduled in any one of these locations during an individual shift.

Responsibilities :

• Conduct annual home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
• Provide education on medical conditions and medications
