Physical Therapist (Home Health) – Pay Per Visit

Location : Willing to Relocate to Napa County / Opportunities also available in Sonoma County & Santa Rosa)

Work Setup : Pay Per Visit

Rate : $130 per visit (based on visit type – Start of Care, Discharge, Routine, etc.)

We are seeking a licensed and motivated Physical Therapist (PT) to join our home health team .

In this role, you’ll provide personalized, one-on-one care to patients in their homes — helping them improve mobility, regain independence, and enhance their overall quality of life.
