Job Title : Registered Nurse Case Manager Profession : Case Manager RN Specialty : Level 2 Case Manager RN Duration : 13 weeks Shift : Monday to Friday Hours per Shift : 8 hours (8 : 00 AM - 4 : 30 PM) Experience : Minimum of one year as a clinical nurse in a hospital required, and nine months experience in home care preferred License : Current RN License (New York) required Certifications : BLS required Must-Have : Certified Home Health Care Experience required OASIS experience required Telemetry experience required Strong critical thinking skills and ability to follow standards / procedures Strong interviewing, clinical assessment, and communication skills Ability to exercise initiative and independent judgment Current driver's license required Description : Develop and revise treatment and discharge plans in conjunction with the patient, family, physician, and care delivery team. Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with defined timelines. Process orders and notify the physician of patient needs and changes in condition. Provide an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiate appropriate preventive and rehabilitative strategies. Provide nursing services requiring specialized skills. Consult with and educate the patient, family, and other team members regarding disease processes, self-care techniques, and preventive strategies. Supervise LPNs to ensure patient health, safety, and compliance with the plan of care. Direct, coordinate, evaluate, and supervise the quality of patient care services provided. Interact with patients, physicians, referral sources, and others to foster positive relationships. Support customer-focused planning and case management designed to meet individual health and service needs. Complete all appropriate clinical records needed for compliance with state and federal legislation. Maintain current knowledge of all community-based services available. Demonstrate flexibility in visiting patients based on need. Ensure problem resolution at the point of contact. Attend patient care conferences for assigned cases. Monitor results, progress, and take corrective action on quality indicators. Understand the importance of the RN function and its impact on various aspects of care. Participate in peer review and quality assessment and performance improvement initiatives. Promote professional growth, learning, trust, and mutual respect among all employees. Participate in education and training programs as needed. Demonstrate commitment to professional growth and competency. Perform other duties as requested.
