POSITION SUMMARY :

Under the direction of the Director of Physical Therapy, this position supports the work of the Y, a leading nonprofit committed to strengthening community through youth development, healthy living, and socialresponsibility. YMCA Outpatient Physical Therapists will provide rehabilitation services within the clinic in amanner consistent with our mission and values. Responsibilities include, but are not limited to, evaluationand treatment of patients, regular communication with referral sources and modeling professionalbehavior. As an Outpatient Physical Therapist, you'll work closely with patients ranging from adolescents

to seniors, employing evidence-based practices to help them regain mobility, reduce pain, and enhancetheir quality of life. You'll collaborate with a multidisciplinary team, including physicians, healthcare staff,and other therapists, to create personalized treatment plans tailored to each patient's unique needs. ESSENTIAL FUNCTIONS :

The Y : We're for youth development, healthy living, and social responsibility.

• Conduct initial patient assessments and evaluations.
• Develop and implement individualized treatment plans.
• Provide hands-on therapy and therapeutic exercises.
• Educate patients and their families about treatment plans and home exercise programs.
• Monitor patient progress and adjust treatment plans accordingly.
• Coordinate with other healthcare professionals to ensure comprehensive care.
• Maintain accurate and detailed patient records.
• Utilize various physical therapy techniques and equipment.
• Ensure patient safety during all therapy sessions.
• Stay updated with the latest advancements in physical therapy.
