Job Title : On-Site Primary Care Physician – Assisted Living Focus

Compensation : $225,000 – $250,000 annually

Since 2016, Twin Cities Physicians has provided primary care to thousands of residents in assisted living, memory care, and transitional settings. Our providers build long-term connections with their patients and lead efforts to reduce unnecessary hospitalizations through collaborative care.

Position Summary :

We are looking for a Physician (MD / DO) to lead rounding efforts in assisted living settings throughout the Twin Cities. You’ll develop care plans, engage families, and collaborate with interdisciplinary teams—all in a flexible, mobile, and high-touch practice environment.

Key Responsibilities :

• Visit patients on a regular schedule in designated senior living communities
• Diagnose and manage both chronic and acute health conditions
• Support end-of-life planning and advanced directives where appropriate
• Collaborate with on-site teams to improve health outcomes and avoid unnecessary transfers
• Foster meaningful patient-provider relationships
