Texas Licensed Care Manager (RN)

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

Evaluate member needs and recommend care plans to address barriers to accessing appropriate care

Develop ongoing care / service plans and collaborate with providers to meet members' unmet needs

Provide education and resource support to members and caregivers regarding healthcare options and benefits

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in care management

RN - Registered Nurse State Licensure and / or Compact State Licensure required

Must reside in Texas as the position is remote
