Registered Nurse – Home Health Case Manager

Location : Montgomery County, MD & Northwest Washington, DC

Schedule : Full-time

About the Role

We’re hiring a Home Health RN Case Manager to provide in-home clinical care to patients in Montgomery County and select zip codes in northwest Washington, DC. This role offers a dynamic mix of patient interaction, autonomy, and community-based care in a fast-paced environment.

Clinicians use their own vehicles for travel between visits and must maintain current auto insurance. Mileage reimbursement is provided from the first to the last visit of the day.

Orientation includes approximately five in-person training days at the Baltimore office, with the remainder conducted virtually or in the field.

A productivity-based point system is used to structure weekly expectations :

• Full-time : 25 points / week
• Part-time : 18.75 points / week

Point Values by Visit Type :

Territories are designed around the clinician’s home address whenever possible. While most visits are localized, occasional travel outside assigned zip codes may be required based on census needs.
