• Description : Under the direction from the clinical chairman provides outpatient care for patients primarily the ages of 65.BID DUE DATE : 7 / 18 / 24 5pm ESTSchedule / Availability Requirements : The clinic is open Monday through Friday 7 : 00am – 5 : 30pm. The preferred schedule would be 4 days a week 10 hour shifts- with a day off on Wednesday or Thursday.Will be taking call about 2 weekdays and 1 weekend per month / will only be taking calls never called back inWeekday Call 5p-7aWeekend Call 7a-7aAssignment will be year long no less than 6 monthsDetails : Facility Location : IM1 Internal Medicine 3120 Clearwater Dr. Prescott AZ 86305Setting : Standard Ambulatory ClinicDuties : Acute care Annual Wellness Exams Follow Up Visits Surgical Clearances Hospital and ER Follow Ups etc. triage calls and after hours with nurses.FTE : 1.0Required Procedures : Wound care ear lavages sutures cryotherapyEMR : CernerSupport Staff : Medical Assistant Front office Assistant RNReason for Coverage : Support with access issues due to departing providerAdditional Details / Questions Answered by Facility : Approximately 15-18 patients per day to see Acutes Annual Wellness Exams Follow Up Visits Surgical Clearances Hospital and ER Follow Ups etc.First day info : First day consists of hospital orientation second day is Cerner training with Provider Concierge / IT third through fifth day is shadowing elbow support will be provided the first two days seeing patients.Credentialing Information : Timeframe : 90 days after receiving completed application Completed Application needs to be turned around in 5 days or lessRequirements : AZ License MD / DO Med MalOther Credentialing Requirements : CV must be in chronological order beginning at Med School no gaps over 60 days without written explanation 3 referencesCredentialing with other facilities?
