Position Summary

The Physical Therapist (PT) at Mountain View Health and Rehabilitation is responsible for evaluating, planning, and implementing physical therapy treatments to restore or improve resident mobility, strength, coordination, and function. The PT collaborates with an interdisciplinary team to ensure individualized, compassionate, and evidence-based care that enhances each resident’s quality of life and independence.

Essential Duties and Responsibilities

Evaluate residents’ functional abilities and create individualized treatment plans based on medical conditions, physician orders, and resident goals.

Provide direct physical therapy services including therapeutic exercise, gait training, balance and coordination activities, pain management, and functional mobility training.

Monitor resident progress and modify treatment plans as necessary to achieve optimal outcomes.

Document evaluations, treatments, and resident progress in accordance with facility policy, state, and federal regulations.

Participate in care plan meetings, discharge planning, and interdisciplinary team discussions.

Educate residents, families, and staff regarding therapy techniques, home exercise programs, and safety recommendations.

Supervise and direct Physical Therapy Assistants (PTAs) and ensure delegated tasks are performed safely and effectively.

Maintain compliance with all applicable CMS, state, and facility standards, including documentation and billing accuracy.

Qualifications

Graduate of an accredited Physical Therapy program.

Current Montana Physical Therapist license in good standing.

Experience in a skilled nursing or long-term care setting preferred.

Strong communication, assessment, and time management skills.

Ability to work collaboratively with a multidisciplinary team.

Physical Requirements

Ability to lift and move residents safely and effectively.

Frequent standing, walking, bending, and reaching throughout the workday.

Must be able to meet physical demands consistent with resident care delivery.
