The RN is responsible for assessing, planning, implementing, and evaluating individualized nursing care to support patients in achieving their optimal functional and physical recovery. This role requires flexibility, clinical competence, and the ability to float to other units as needed. Key Responsibilities : Provide skilled nursing care to patients in a rehabilitation setting, focusing on recovery, mobility, and functional improvement. Assess, plan, implement, and evaluate individualized care plans based on patient needs and goals. Administer medications and treatments in accordance with physician orders and nursing standards. Monitor and document patient progress, reporting significant changes to the healthcare team. Collaborate with interdisciplinary team members, including physical therapists, occupational therapists, and physicians, to promote optimal patient outcomes. Educate patients and families on care plans, rehabilitation processes, and discharge instructions. Maintain accurate and timely documentation in the electronic medical record (EMR). Float to other units as required to support hospital staffing needs. Adhere to all hospital policies, safety protocols, and regulatory standards. Provide compassionate, patient-centered care in alignment with the facility’s mission and values. Qualifications : Education : Graduate of an accredited nursing program (ADN or BSN). Licensure : Current and valid Registered Nurse (RN) license in [State]. Certifications : Basic Life Support (BLS) certification required . Experience : Minimum 1 year of Med-Surg nursing experience required . LTAC (Long-Term Acute Care) or SNF (Skilled Nursing Facility) experience accepted.
