Licensed Clinical Social Worker - Case Management

About this Opportunity :

The Licensed Clinical Social Worker (LICSW) in Case Management is dedicated to supporting patients at MedStar Health hospitals by helping them navigate and access essential care services. This role emphasizes the coordination of care through identifying eligibility resources and ensuring consistent follow-up to help patients overcome barriers to care and achieve optimal health outcomes.

Key Responsibilities :

• Contribute to department goals and comply with policies, quality standards, and safety regulations.
• Maintain compliance with relevant rules, regulations, and MedStar Health policies, and report any compliance concerns promptly.
• Engage in grant writing and follow-up activities with partner organizations.
• Document patient interactions and treatment plans effectively in MedConnect (EMR) in alignment with funding source requirements.
• Collaborate with community providers to enhance service delivery and identify additional resources.
• Assist in developing self-management curriculums and guide patients for effective implementation.
• Coordinate appointments with behavioral health providers and facilitate screenings and support services.
• Manage transitions of care for eligible patients and work closely with community resources.
• Guide patients in self-management strategies and provide education on treatment adherence.
• Coordinate services with navigators, community outreach specialists, and other healthcare providers.
• Engage with community groups to provide educational programs and facilitate support groups.
• Conduct medical case management including adherence to treatment, harm reduction education, and psychosocial support.
• Maintain accurate records and data entry as required by programs and funders.
• Provide leadership and support to non-licensed staff and supervise social work students, if applicable.
• Participate in multidisciplinary teams and contribute to quality improvement initiatives.
• Perform other related duties as assigned.

This position offers a competitive salary range of USD $72,758.40 - USD $123,260.80 per year.
